ESA form: can I tick 3 boxes for 1 question?

Hi I was advised to keep all queries to the one thread.

I have a variable conditions and all tasks I have problems with I am replying that:

"I have problems with [this activity] for the majority of the time, better days & bad.
"

I would also like to say for some that: "I never have no problem with this activity"

Is that sensible?

The double negative might confuse, how about "I am always affected in this way" or something similar.
