Are you saying that you provided the words for the ESA113 and the GP has stated this on the form?
If you are then I would expect all those reviewing the claim to ignore the contents as tainted.
If the GP has used your words but simply signed it, then it should not be a problem.

It's in a similar vein, but to be honest worse.
I'm very surprised that a GP would do this without thinking of the consequences. If they had just copied what you had written onto the form itself it would not have been a problem.
