Evidence from the GPs was created by Orangeblossom
Hi, can i ask about getting a print out of conditions and medications from the GP?- do you have to ask the GP or just ring the surgery for this? My Gp has done this in the past to go with a ESA 113 form but think it may be better to have one more recently updated, for PIP.

The simplest thing is to ring the surgery and ask.

Thanks, the reason I asked on here is I struggle on the phone. So just wondered if anyone had done this.
