E.S.A.50 where should form be sent was created by macca7894
Hi all just completing my E S A 50 form and i have noticed the return address is Arden House Assessment Centre not D. W. P. does this mean i am getting sent straight to face to face assessment. I have been on Invalidty Benefit then INcapacity Benefit and this is the first E S A form i have been asked to fill in i have been sent to lots of assessments in the past but not for transfer to E S A many thanks

ESA50's are sent our and returned to the Assessment Provider, Maximus, they will then decide whether a face to face assessment is required or not.
