Hi there I am on ESA. If I'm re-assessed, will I receive the esa50 form or a something different? I've got a vague notion at the back of my mind that a short version of the form has been discussed here... Many thanks

Most claimants receive an ESA50, if you are in the Support Group then there is a small possibility that you might receive an ESA50N, this is a shortened ESA50 that only covers the ESA Descriptors.
