I am in the process of filling in the form but am seriously disabled and unable to write more than a few words - confirmed by my GP in his letter. I can however use a computer slowly but effectively. I phoned up and asked if I could have someone else fill in the form but let me supply the extra details to each question by a printed page. I hope this is clear I would have someone write "Answer to question Q8c is on printed page headed Q8c Answer". After some discussion the last person I spoke to said this was ok. Do you agree please as I don't want to lose my claim as I am simply unable to fill in the form and could only write a few words instead of answering each question FULLY. There are excellent reasons why i don't want others to know full details of my illness or my prognosis.

It is perfectly OK for you to type your answers and attach them as extra pages to the PIP2, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
