Help required re typing answers to PIP form please was created by MD
HiAs there is no online Pdf form 1003, is it possible to type out all the answers to questions on separate sheets, and staple them to the form?
If so do I just number, and head pages with claimant details and the question number, or do I need to also mark them as Q15 Additional Questions and also the question numbers?
I would of course tick the boxes on the original form and write out where the answers are.????
Any ideas or template. available that I missed?
I am filling out 1st PIP from DLA form on behalf of my husband, as his appointee.
I have really terrible handwriting and it already looks like dogs breakfast on first 2 questions.
As it is only 3 weeks before the deadline( 6th Sept) I do not have time now to ask for an electronic version. ( That is if they will give it to me anyway) It did take a while to get in the post from the claim office.

Hi Miz
I'm sorry but there is not an electronic copy in the public domain, however, it is perfectly OK for you to type your answers and attach them as extra pages to the form, just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
Don't forget in the question answer box to write 'see page X'
If you have any medical evidence - then don't forget to include it and write a supporting letter as to what help and assistance you give to your husband, put your name and address on the supporting letter and sign it.
It is important that you understand that PIP is a very different benefit from DLA, the intention of the Government when they introduced the new benefit was to reduce the disability benefits budget by at least 20%, so the criteria is a lot stricter than DLA and the assessment process is now task orientated.
Make sure you understand what the Descriptors say, that you target the Descriptors that you believe that you can reasonably meet and that you follow the advice in our Claim guide to help you answer the questions.

I will make up a template of sorts and number it as you suggested. I did do a similar thing on his prior dla and sea forms. And yes, the questions are very differently worded, I shall pay close attention and go over everything multiple times.
I did the test run rather conservatively, and it looks as if he has more than enough points, but I remain a bit concerned.
It has been many years since his haemorrhage, and although the gp will give a factual letter relating to his condition and medication, that is all he will do. We did move across the country and therefore changed gp also.
I am both wife and full time carer I do not have support from proffesionals, or anybody else, so I will take time to do this as well as I can.

Thought i'd post my question here as its very similar.
Is it ok for me to print out the answers, cut and glue them in the relevant sections of the form?
As this is easier for me and clearer then additional pages/referencing.

Hi Miz, just a thought, have you linked up with your local Carers' Association? The support they offer varies widely from county to county. Many have got NHS funding for supporting carers and will offer you an assessment as to what help you are entitled to. They may point you to any local source of help with benefits claims, though these are scarce. Some have negotiated discounts with local shops and services, some offer free or cheap relaxation therapies eg massage. Worth checking.

Hi Gentle
I can see no reason as to why you cannot print and glue your answers into the relevant sections. We have had members on this forum who have informed us that they printed and glued the answers on to the form and had no issues.
What I would say is that you need to make sure that the glued answers are well stuck down as your form goes through a machine to scan your answers on to the system.
