Electronic PDF form for PIP? was created by Finni
I received a PIP application form and also downloaded the B&W guide to making a claim. I will need my support worker to help fill it but she's been told to isolate while waiting for her Covid test result.
In the meantime I want her to make a start on the form. The closest thing is this old 2018 form on Gov.uk. Is a more recent one available?

In fact, is it possible to submit a special electronic form for PIP filled in by myself, as I did with ESA? That way my support worker and I could pass it between us and then submit the result.

I'm not entirely sure where you are in the process, whether this is a new claim, reassessment of renewal.
The PIP1 that you mention in your post is used to make a new claim for PIP, you can submit the form or you can make a claim over the phone and the DWP will fill in the form for you.
If you are talking about the form to describe your limitations then this is a PIP2 for a new claim or a renewal, there is a link to this on the same page as the PIP1.
If it is a reassessment then the form is an AR1 Review form.

None of the forms are available in electronic format unless specially requested from the DWP but you can provide your answers for the PIP2 and AR1 on separate sheets this would allow your support worker to participate in the process.

This is a brand new claim for PIP. I stopped receiving DLA about 2 years ago when it was transferred to PIP and I was too ill to apply at the time.
I have recently received:
(1) 40-page form called "How Your Disability Affects You" (reference "PIP.1003")
(2) "How Your Disability Affects You - Information Booklet" (reference "PIP2").
(3) A one page overview about returning the form and suitable evidence.
(4) A envelope for reply.
Am I missing a form?

That sounds like everything, the PIP.1003 is the PIP2 form.
make sure you use our PIP Claim guide to help you complete the form.
