Cancelling appointments due to anxiety was created by Peawack

I have had to cancel psychology therapy due anxiety when physical treatment at another hospital coincides. Just too much to handle. Should I mention this in my PIP review form ?

If you are not getting the help you need because of a clash and increased anxiety - yes I would mention it.
