Hi Gordon i am receiving PIP standard mobility standard daily living i sent my review form back and i have just had a text massage from PIP saying A telephone consultation has been booked for 5 November I claim PIP for Depression Anxiety i have had a triple heart bypass i have got unstable angina nerve damage in right leg after bypass lower back pain balance problems i have just been diagnosed with ME Chronic Fatigue After i have had my phone consultation i will want to request a Assessment report Who do i phone to request this form from is it PIP i phone to ask and is there any advice you can give me to get through this phone consultation and do you know if the pass rate is good thankyou

Hi christine

I've moved your post to its own topic, we ask members not to post their questions in other member's topics as it can lead to the wrong advice being given or taken.

If you have a look at our PIP guide at the link below you will see that there are 9 pages which include advice on telephone assessments. Yes, a lot of the questions are those that would have been asked if you had a face to face assessment.

Some practical advice;

- have a glass or better still a jug of water available if needed so you don't get a dry throat and lose your voice!
- make sure you are seated in a comfortable chair, you are going to be there a while.
- create a bullet list of your conditions you want to be covered and how they affect you.
- have the papers available, so you will need to be sitting at a table or similar, if you need to refer to a particular supporting document.
- pen and paper in case you need to make notes
- if you can't hear or understand what is being asked then ask them to repeat the question.

You should contact the DWP for a copy of the assessment report (PA4). You should phone them after 48 hrs after the assessment and again follow up the request in writing. We have heard on the forum members being informed that they cannot have a copy of the assessors report until a decision has been made, this is false information and you may want to speak to either a manager or team leader.
