PIP Review Form was created by i8plumjam
I have received the review form to fill in but have been told NOT to send in any extra documents that they may already have. My award was 4 years ago and I recently had an ESA face to face last year in which case extra evidence was sent then How do I know what to send if both claims are separate and years apart? Will they all be grouped together or should I send everything I have regardless?

This is your claim and you are entitled to send any information that you believe that is relevant to it, the DWP cannot tell you otherwise.
You can send the ESA info but you will need to explain why it is relevant to your PIP Claim, there are areas of overlap but they are limited, there is a document in the PIP area that explains where the Descriptors are similar.
It is very unlikely that the DWP will source your ESA information when they make a Decision on your PIP.

One more thing. When my last PIP was entered, they didn't give me the points for enhanced mobility even though Id had a lifetime award on DLA. I appealed for a mandatory reconsideration and sent a letter along with a GP back up letter, then they overturned the decision. How do I address this on the review form? Will they see the original form and the letter of appeal or what?
I don't know what to do and Im extremely anxious and upset. Please can you advise.

As Gordon has already told you, you are entitled to send in any piece of relevant information that you want. You can either send in a copy of your GP's letter and a copy of your MR or you can simply address it on the form. There is a space for you to add any additional information - or in the box that covers mobility - put in See additional letter X and add additional pages. There is no right or wrong way to do it. If it is going to worry you that you have not sent a particular piece of information - then please send it.

Thank you. I will do as you suggested and just send everything I have regardless of whether it will be duplicated not and hope for the best!
