Seeking a copy of GP report form for PIP

Is there a copy available of the GP Factual Report Form mentioned in the B+W Guide to applying for PIP?

My GP is helpful, so I would like to draft out the points about my illness he should include. I'm aware the section the B+W PIP guide provides some headings under "What's In a GP Reort Form" but the actual form would be more useful.

On the B+W "PIP Resources" page I see a completed 23 page "Consultation report form – PA4 V3" but it doesn't look like something a GP would fill in.

Can someone kindly steer me to a copy of the form the GP gets. Thank you.

No, we don't have a GPFR form, it's just a form with some standard questions and usually some specific to the claimant, the black form does not make any reference to the PIP criteria so will not be of any help with your GP.

However, there is a GP Notes document in the PIP area which has all of the information you require and can either be completed by your GP or used by them to provide their own response.

The form sent some time ago to my GP for ESA was very basic and had precious little room to write my medical difficulties

By contrast, that GP Notes form for PIP you mention is vast! Surely a GP isn't asked to go into that level of detail for PIP (with or without that form to guide them) ?

The GP Notes document is 14 pages, an explanation page and then one page for each PIP activity, all the GP has to do is to tick which Descriptor they think is relevant and then sign it, the idea is that you only present the pages that you think are relevant. It should be quicker and more relevant than the GP producing their own letter.

Gordon, thank you for your kind advice.

Please excuse me if I am sounding a bit thick but exactly how do I use that GP Notes document? The first page suggests the form is given to the GP and he fills it in but then what? Does the GP return it to me after he's filled it in because that seems unproductive.

I was thinking of having a form which I fill in myself to inform the GP the exact difficulties I encounter. My GP doesn't know or can't recall all the details of my life and giving him a list would help jog his memory.

Presumably he will be asked by the DWP to fill in something similar and can use my pre-filled form as a template for his response.

Is my proposal going against the grain of how PIP is assessed by the GP and will actually mess up the process?

Thank you for any advice. Sorry to be so new to this.

Hi Finni

As Gordon said in his post - you present the questions that you think are relevant. General doctors are not going to have any specific facts about your ability to cook a meal (it's not something they have ever observed), but they can comment easily on mobility. You say that it's unproductive, but it guides the doctor to write to the specific criteria which is what PIP are interested in. However, you don't have to do that - plenty of people let their GP write a letter in support of their application - some are very good and some less so.
If you have a good relationship with your doctor - you can always draft the type of letter you need and see if they would be willing to write a similar letter as long as you know what it is that you need.

You say in a previous post that the DWP contacted your GP for your ESA claim. Do not assume that they will do it again for PIP. Only a small number of forms are sent to GPs. The majority of claims are decided only on the information that the claimant sends in themselves along with evidence they have gathered from medical professionals.
