Thank you, I've just written another essay ( I don't know if the subject is similar enough to merge?).
Re the grip, it has got a bit worse, but I don't have any evidence apart from the last assessors report, I've mentioned it as part of much whole pain physical issues to doctors etc but haven't gone in just go Speak about it and don't have a letter or anything about it.
I explained to the telepjone assessor that my partner cuts up my food, due to pain & grip, I said I could use a knife (not for cutting) , fork & spoon, . That my partner often puts it in a bowl and that I often will avoid food (s) because of pain.
They have put A = 0 points
" Although she is on medication for pain and under the psychologists to help with pain management and she reports poor grip in the hands.
Functional history report" says in there: her partner cuts her food " she is able to use two crutches (I'm on elbow crutches, or use rails/ walls in house) and able to use knife and fork to eat which would suggest by all probability she has adequate grip in her hands.
It therefore would be reasonable to suggest on balance of probability she would be able to carry this out independently safely in a timely manner reliably to an acceptable standard the majority of days."
The difference to that compared to somebody who has seen me is so night and day. Also she kept saying how fine I was, I couldn't remember names of things repeatedly, my partner was calming me down (I don't know if she heard though).

Is there a sample letter anywhere?
Do I just send it to the decsion maker at Wolverhampton?
Is my word or my original form good enough as evidence, e.g. On my form, I stated that my partner had my meds because of my history with overdose - I don't know where they are.
He reminds me to take them, and wakes me if sleeping ( due to my eratic sleep patterns)
He keeps them in a dosage box and if I was going out, for example to the hospital with my mum, he would give me the days strip. On the phone I couldn't remember the name for this and said he keeps the tablets in the box with Monday , Tuesday etc on it and he'll give me the " days strip". as above.
I got 0 points because not under mental health team! Even though I'm under a clinical physiologist working on both my pain and mental health since 2017.

It is one of the problems with a telephone assessment - that someone can't your difficulties. Remember the assessor is only offering an opinion - hence their comment about your use of knife and fork and crutches. It's a common theme for them to say that people have sufficient grip if you use these things. Yes, you could use at evidence your previous report where you were seen face to face.
There's not a sample letter before writing for an MR. If Wolverhampton is where your original form went - send it there.
