Hi all my wife had her telephone consultation on 3rd February she was absolutely nervous and very anxious she answered questions as she had all the paperwork in front of her I had to prompt her on lots of occasions we recieved letter through saying standard living and standard mobility I had recorded the consultation and found in the letter about it totally wrong about my wife as the lady asked about medication she cannot deal with it I do in a disease box they said in letter she can managed her medication and too cut along story short goes on like that so asking for MD have downloaded the form to fill out theres a question which asks have we any information to be seen or heard ? Do I disclose that we have a recording or put that in the appeal thank ypu

If you have not already done so, please do have a look at the guides in the section about MR and appeals here

I am not clear whether or not you have requested and received a copy of the PA4 form, which is the report which the assessor wrote immediately after the assessment and will contain recommendations about points.
If you have not got it, you need to do so. You can get it by ringing up the DWP and asking for it.
Your rebuttal and contrary evidence to that form will be the main substance of your request for MR. (You do not need to use their form if you don't wish to - a simple letter surfices.) By all means say that you are including additional evidence that what your wife said with regards X & Y was incorrectly recorded by the Assessor on the PA4 you include a transcript of the relevant part of the assessment taken from a recording which we made of the whole assessment. You don't need to send in the recording, but make sure you keep it, ideally with a back up.
it would be remiss of me not to point out that by asking for the DWP to reconsider your wife's award there is a risk that they could decide that she was actually awarded too many points and deduct some. It hardly ever happens, but it is something you should be aware of.
