Seek blank PIP form sent to GPs (like PA4 V3)

I applied for PIP and had a telephone interview. Later my GP phoned to say he's recieved a form and he asked me to provide an outline of the answers so he can base his reply on them.

I jotted down some of the questions (but not the instructions to the GP on what to put in each section). The questions are vaguely similar to those on "Personal Independence Payment Consultation report form – PA4 V3".

Where can I get a copy of this form?

---- start ----
(1) What are the disabling conditions
(2) History of these conditions
(3) Symptoms and variability
(4) Relevant clinical findings
(5) Treatment which is current, planned and responds(?) to diagnosis(?)

(6) Effect of disabling condition on day to day life:
(a) ability to manage their health condition and treatment
(b) communicate
(c) walk or move around
(d) get somewhere on their own
(e) make simple decisions
(f) prepare, cook and eat food
(g) wash, bathe and use toilet
(h) dress and undress

I went to the claims section but saw the "PIP GP Notes" which looks very useful for em to communicate with my GP.

However, that is not the form he has been asked by the DWP/PIP to fill. This is what I would like to see. Do you have any idea where I might find a copy of that?
