Do you know why the assessor thought you didn't go out at all? If you do go out and it is with help - then in your mandatory reconsideration you can explain what you can manage and get a letter or letters from the person/people who help you and can explain your difficulties. (These can be friends or relatives - they don't have to be professionals)
