PIP Award Review AR1 UI - How to provide answers

My circumstance have changed and speaking with the DWP - PIP - Team, i was sent a PIP Award Review form AR1 UI.

Getting to the point of my question, in the form they allow 2 to 3 lines for a answer, now i have studied the PIP guide from B&W and have pre-written my answers on my own paper and tried to include all the relevant information regarding my conditions that relate to that task or activity.

I plan on re-writing my answers once we have rechecked them to make sure we have gotten to the point as in why, how, when & what my conditions allow me to do or not do.

Can i use their form AR1 UI answer spaces and direct them to see the attached page that i plan on stapling to their page that goes into detail about me and my conditions, as there is not enough room for me to reply and i do not plan on stating "No Change" as a answer.

I will mark the top of each of my pages so it is clear what the page relates too.

Example: top of my page will state:
Section 3 - 1. Preparing food and cooking, then

Tell us if something has changed and approximately when.
My answer will go here and i will do the same for the next two fields.

I am already being pressured to return the form, as they have sent me a reminder letter and it states on the form that i have until 23rd July 2021 for the form to be returned to them.

Anyway, that is where i am at right now and i plan on returning the form sometime in June, once i am sure i have done it correctly this time.

Our PIP Claim guide covers completing the AR1 Review form (1043 Review), remember, whatever the form says you are being assessed against the same criteria as before. If you follow the link below it will tell you what you need to know about completing the PIP review.

It is perfectly OK for you to type your answers and attach them as extra pages to the AR1, just make sure that each page has your full name and NINo at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X. If there has been no change in your circumstances tick the relevant box and then write below please see page ‘x’ of ‘y’, you do need to explain your condition even if it has not changed. If your condition has changed, then tick the relevant box and explain what has changed and how it affects you.

Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
