Pip report evidence for lcwra was created by Editor68
Hi,
I was awarded high mobility and care in 2018 for Pip and have a full copy of the assessor report which I requested. I will of course cross reference this while applying for lcwa.
However can I send the full report or parts to show how my mobility is restricted as evidence as this was done by a nurse.

As long as you reference it - you might as well send a copy of the entire report (as long as it's all helpful!)

Hi Bis,
any tips on how to do referencing on say a PIP review form,
example i want to use my ESA SG assessors and DM's report and give ref to it. As its vital towards my mobility PIP activity 12.
Has anyone or could it be possible if someone could put up a template on here, i find it would be helpful to us all. Especially if we need to add lots of other information ie med records etc to our ESA or PIP forms.
Referencing may come simple for some but for others its difficult.

Our PIP Claim guide covers completing the AR1 Review form (1043 Review), remember, whatever the form says you are being assessed against the same criteria as before.
It is perfectly OK for you to type your answers and attach them as extra pages to the AR1, just make sure that each page has your full name and NINo at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X. If there has been no change in your circumstances tick the relevant box and then write below please see page ‘x’ of ‘y’, you do need to explain your condition even if it has not changed. If your condition has changed, then tick the relevant box and explain what has changed and how it affects you.
Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

I referenced parts of my ESA report for PIP by quoting the page number and then line number of the particular statement I was highlighting.
I do not write anything on the forms other than see page x of y. All my information is typed and I put all the evidence as appendices at the end. We also did this for my Husband's claim and the assessor complimented us on it, saying it was the best claim she had ever seen.
