Hi I've just joined as I need to complete my childs dla renewal. The form has changed and not based in minutes and endless tick boxes so wanted to make sure I fill it in the best I can with a guide from here. I can't find a child renewal form guide. Is there one on here please

I'm not aware of a Renewal form and a Google search hasn't uncovered one either so I am not sure how well the guides will fit with the form you have but it is important to remember the assessment criteria, whatever questions are asked, is the same from when you originally applied.
