PA4 form was created by Caval1er
I've got a copy of the PA4 and they have accepted and awarded D needs prompting for preparing food but have awarded 0 points and advised I can take nutrition un aided. To me this is a contradiction, ie if I need prompting to prepare a meal then how can I take nutrition unaided without prompting??? Am I missing something on how to interpret this? Thanks for any help.

No, you're not misinterpreting it. It is a contradiction and unfortunately reports quite often have them.

Just to clarify.
There is a distinction between the two.
The first is about your ability to prepare and cook a simple meal, there is no requirement for you to be able to eat the meal when it is ready.
The second is about your ability to consume food and drink, there is no requirement for you to have prepared or cooked the meal, in fact, it could be a ready meal or a takeaway.
If you have problems with both then you need to explain your issues separately.
