New Claim for PIP: Online version and Evidence was created by devonrex
Hi
I am in the process of completing a new claim for PIP on the online application and have some questions regarding the form and what evidence to include.
1) relating to evidence: should I still create an index like I would have with a hard copy and if so,
-should I rename any evidence that I upload to a new filename that is more fitting or does the DWP not even see what the filename is called? If not, I have got confused on how to upload some, scan the rest and then create an index that makes sense relating to each condition.
Also, where I would have written by hand on all the evidence hard copies, my name, nin and dob, should I ensure I add this to everything I upload? Some of them are pdfs.
2) could I be at fault for sending 'too' much evidence? For example, this past year, I've had loads of tests due to a lung condition and I have test results, letters, discharge notes... should I just send the latest report that includes everything in it?
3) in the paper version, where it says medications, I was going to include all the pain med increases / changes to a higher pain medans side effects. On the online version, it has medications with each condition, should I still describe all the previous changes of meds? I also have evidence from the pain clinic for some of the changes in letters to my gp so if I upload this, do I rename what it is in the filename or in the upload itself or both?

1) I haven't had experience of using the online form - so I can only hope that a member might be able to tell you what they have done. I would want to create some type of index if possible. I would always want my name, DOB and national insurance on any page I was uploading.
2) Too much evidence? The DWP has never made it clear how much evidence you should send. I believe you should send what feels right for you. I have known people to send anything from 1 page to 700 by post! Obviously, 700 was extreme, but the person thought it was necessary.
3) I can't tell you whether you should rename a file. It just needs to be as clear as possible to what it is. It's up to you whether you tell them about all the changes - you may want to do this on a separate piece of paper.

Thank you for your reply.
I know there is an easier way but my brain doesn't seem to let me know how to go about it. I've got until 2 August.
My dilemma is:
2010 to 2019 medical records / letters etc are in my one drive as individual pdfs.
2020 onwards medical records / letters are originals that I have scanned and saved as individual pdfs.
None have my name. NIN, date of birth or page number.
I don't know if I should collate everything above and try and save it into one big doc with an index at the front.
Or upload them individually with an index.
Ahh hopefully someone will see this post and advise me on how they completed the evidence part of the online application!

I am finding the guides extremely difficult to follow with an online pip application. The medication/health condition is all in one box instead of split and until you start filling the form in you do not realise you have to save and next to add a second condition if you have one. Does anyone have any information that I could read that would help me complete the online version please? I am really struggling. The guides are fantastic for the PIP questions however, it is just the health conditions and evidence that is not clear.
