Hi there
Could anyone tell me if this is a normal thing to find on the PA3 report?
Under “Type Of Professional “ .. it just states :- Health Professional.
So I have no idea in which profession this HP works in, is this normal or should they state which profession they work in?

I have seen some reports with just Health Professional and some that have said 'Physiotherapist' or "nurse' etc. I don't think people care if they get the outcome they are hoping for. But if there is a dispute over the report and a claimant feels that they want to make a complaint about the conduct of the assessor to a professional body, then it helps to have their exact profession as a starting point.

My PA3 v2 report is poor , in fact I'm shocked the HP got basic information wring, its contradictory to the written facts in my medical report . It had inaccuracies in basic dates even, plenty of evidence left out and not even acknowledging half of what I said, and using the fact I passed my driving test as solid grounds to score 0 and used it to cover plenty descriptors which as a result, I'm zero in all activities. The DM report of their decision is almost identical and their decision was made off My CQ & HP PA3 report, so simply me against HP and DM states they got enough off the HP to make a decision.
Both reports relied heavily upon the “passed their driving test” stunt as strong supporting evidence against me on multiple activities causing the same results, there is not much explanation as to how they came to the decision. I never mentioned passing my driving test as it was some 20yrs ago, so it’s irrelevant to my claim , I've only stated I don't drive anymore for various reasons related to my complications, yet thats what they came out with.
The HP also had to refer to a senior DA for advice for on what to do. My GP has told me.. as a general practitioner they dont have enough knowledge on my particular issues regarding my MRI scan, GP admitted didn't know what it meant, but did make the effort to look into it to get some sort of understanding for my next visit.
So I would like to know what profession the HP works in as my issues are of a higher level of understanding , its for a specialist as I have surpassed GP level of help.
I clearly have to go all in, and enter the necessary stage of MR. Ive only got 6 working days btw, yes i had the letter late.
What Id like to know is, Is it still possible to request what profession the HP was ?
And can I request a transcript of calls of my very short phone call assessment, and a transcript of the call made to the senior DA?

I would not worry about what profession the HCP was, I would just concentrate on your MR.
Now you have the assessment report you will have a better understanding of how the DWP Decision Maker has come to their conclusions and you will now be able to argue against them.
I usually use a highlighter to highlight any inaccuracies, once you have gone through the report you then set out why you are applying for a MR and go through each descriptor one at a time why you disagree with the assessor and try and match it with any supporting documents you may have sent in.
Understand that the assessor is offering an opinion when they say 'in my opinion' you can do the PIP activities, tackling these head-on is unlikely to be successful, you need to show that the basis of that opinion is founded on incorrect information, so try and refer to your evidence to undermine what they have said, remember they are just an opinion which is not based on fact.
Also, try and stick to issues that lead to your scoring points, I often hear members referring to the assessors spelling, as an example, whilst it may show a lack of professionalism, raising it as an issue isn't going to help you get an award.
Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation.
Lastly, make sure that you understand the criteria that you are being assessed against you can put the best case forward that is possible but you won't score points if you do not meet the PIP Descriptors.
I know you have stated that you do not drive anymore, information about driving, and the correlation with every other PIP activity, LL26 wrote a good post on this issue.

The first stage to challenging a Decision is for you to request a Mandatory Reconsideration, this needs to be done in writing to the DWP, within one month of the Decision, to the office that dealt with your claim, have a look at our PIP MR & Appeal guide; benefitsandwork.co.uk/guides-for-claimants/pip . You can request an out of time mandatory reconsideration with good cause such as waiting for the assessors report PA3.

When you have a better idea of the issues with your claim, come back to the forum and we will do our best to help.

Thank you for all the information, I will follow your guides as best as I can, I'm just worried I wont make the deadline, 6 working days is very short window to prep a MR in my condition even with the help I have.
Could I request MR now so at least im not knocked out,, and just let them know I will be sending in my reasons afterwards?
Due to my condition I don't think I will make the deadline , I took 7 months to fill my CQ, and still continued over the next 5 months sending in more evidence and that is with help, I can only so much at a time,
. Is it possible to send in my reasons after I initiate MR in writing?

Yes you can phone DWP and make the request for a MR, they will ask you the reason why and ask if you have any additional supporting evidence.
