Hi I'm not sure will get a reply in time but just finishing the Ar1 and the new form doesn't seem to have a place where they ask which assessment is best or which dates you can't do for instance if got hospital appt, am I missing it somewhere or do they not ask on the new form I'm really starting to panic any advise would be great. hopefully if written out in time I'm posting off Thursday hopefully and thanks

Hi Fiona

If you haven't sent it - there isn't a specific place. As I said in your other post - you just need to put in a request in the area where they ask if you have anything else to say - or put it on a separate sheet of paper -(with name and national insurance number)
