NHS APP - End dates on lifelong conditions ?

Hi,

This in a round about way may be important when submitting medical evidence for benefits.

I did a deep dive ( well sort of, as my surgery seems to be adding sections fairly frequently to the app, its taken long enough) I was taken aback to find ''end dates'' on my app next to some of my conditions ! It is bizarre, I have complicated long term conditions that do not and never will, disappear. This was the first time I have come across this section on the app. I look frequently as I have so many hospital appointments, blood tests etc. I really need to look into this, as to look at it at face value, it looks like I now am totally free of these conditions, I am not ! Its a bit of a worry that someone could just print off a list from several sections of the app to send to DWP as proof of conditions without really scrutinising what they are sending. To look at mine, how it stands at this time, I would guess three quarters of my conditions are not mentioned anywhere at all on the app, which isn't great, then of the remaining chronic, long term conditions, two have end dates! how, why and when.

I really need to get onto my surgery and see how I/them get to fill in the gaps and tell me what the end dates mean, as I have no idea, which means a DWP assessor would have no idea either and just tick, ''the fine'' box. I was just wondering if anyone else has come across this ? If anyone has, how have you resolved it ? My surgery will not do letters for DWP, they point blank refuse, don't even have a paid service now. If I ask for a basic health record printout it will be the same info that's on the app. I can see this being another rabbit hole to reluctantly climb down at some point.

Hi Cazzy

Hopefully, other members who use the app will be able to give their experience.

Hi Cazzy

I can only share my own observations and experience with the NHS App and GP service.

As an appointee for afew ppl i have had to rely on NHS APP info to understand medical history and evidence conditions. So ive scrutinised stuff quiet abit. I don't ask for letters from GP.

I've been disappointed with the quality of the NHS APP info and noticed gaps too. On each occasion where i have spotted key inconsistency, i have submitted a eCon to query . this has resulted in either amendments to said record or a call from practice mgr to explain or to have a convo on the issue.

eg Psoriasis was not showing up on a patient record. PMgr advised that she could see this condition on her side of the patient record system. the condition was in remission, afew years ago, so not being treated by the GP. But i said it was important for patient record to reflect history clearly as it demonstrates a pattern.

I suspect ‘end dates’, in the records i've seen, indicate when issue is not currently being treated by GP . this might be bec its been treated successfully or being treated by hospital specialist team(outpatient care). Not sure if my logic is correct.

I've also challenged GP records where conditions are categorised as ‘minor’ , but should be classed as ‘significant’ due to chronic nature of condition

I know its difficult to liaise with GP staff and the bureaucracy can drive you nuts.

Just remember you can be selective about info you submit - so read all NHS records before submission to ensure it supports your claim.

I’ve just joined a new surgery and they use Patient Access App. I will need to review the quality of information on there. It should mirror NHS App records. But i wonder if DWP would accept screenshot from the Patient App
