Is it best to make a note of what question you are referring to when typing up another sheet? I am in the middle of mine now and I have started a sheet basically going through every activity and how they affect me but without referencing which question I am answering as I am just making a detailed response of every question as I go along.

I have moved your post as it gets confusing if we are trying to answer a question on someone else's post even if it is about the same thing. We don't ever want people to get the wrong answer.

Yes, you should definitely make it clear which question you are answering on each additional sheet. Assessors have limited time and they won't try and work things out if it's not obvious. It's one of the worst mistakes a claimant can make, assuming that they have answered a question when it has no number or detail against it. it's not obvious to the assessor they won't necessarily go looking for it or make a link to a particular question that isn't stated.

Hi Kelz7683, you're making a good start.
Before sending it in, I'd separate the answer for EACH question onto a separate sheet. Title the sheet with the question number.
I then number all my extra sheets top right, with Doc X of Y [total number]. In the form I just write in each question answer box, "See doc X"
Remember, name and National Insurance Number as a header for every single sheet including eg photocopied doctors letters.

Punch the top left corner and put a treasury tag if you have one, or wool or string if not, through. This makes it easy for the contract scanning workers to remove the tie and pop the whole stack in the scanner auto-feed. I've never had anything go missing by following this B&W advice from literally 2014.

Oh no, I sent my review form off a few days ago - for each question i just wrote 'see attached sheet' and wrote it all on there with writing any numbers. Worried now

Dear Lisa, I've since discovered I'm Autistic plus ADHD so I do tend to go for overkill if I once focus on a task. As BIS says, no help to yourself worrying now.
