And a footnote from experience.
If you can't do the diary your wife can. I successfully did one for our adult child living 'independently' by phoning daily and noting the results in a simple table on the computer.
Columns for date, rough time called, response, what this meant for PIP descriptors.

Hello everyone,
Thank you all so much for your encouragement and nice words
I had my phone call today from the disability resource centre.
Briefly the very nice lady took me through a lot of the same questions that the assessor asked, and then wrote me a very good mandatory reconsideration letter.
After i had sent her my original application for pip she adjusted this letter to include everything.
She then said she would print the letter and send it to DWP for me, so i don’t even have to call the DWP to request an MR!
I phoned the DWP last friday to request my assessors report, (45 mins waiting) which they said would take 7 to 10 working days to get to me.
Considering the dwp took 10 days to get the refusal letter to me and will take a further 10 days to send me the assessors report, i am so glad this place existed to do it all for me.
I just hope she did it right and hope it doesn’t matter that i didn’t wait for the assessors report?
But i guess this will be more useful at the appeal stage because i don’t fancy my chances now for getting a good MR.

you can file a tribunal claim without having to attend stating the reasons your health precludes you from doing so

Don't worry about the PA4 now. If it arrives and there is something on it that you want to dispute - and you haven't had the result of your MR - send it off as an amendment and say you were not able to send it earlier because of the time it took to arrive.

Hi Adrian. I went from zero points to Enhanced Both at Appeal. It can happen. Glad the resource centre has been helpful. Take care.
