No exemptions if they have sent you the above form! Linking Rules for IB have been done away with!

Also, she said there was no need to send in extra information with the form.....Yeah right!! I am gonna include every scrap of evidence I can get my hands on....


In my opinion you can send in a subject access request and get a copy. If I am not mistaken the form (FIT NOTE) was a MED3 I recently got all the information, and I do mean all, that the DWp had on me because they didn't send me a copy of the atos report when requested by phone, twice, I used this form even then I had to email the benefits delivery centre and inform them the report was missing. If you want a copy of the FIT NOTE as you call it specifically request that in the part where the form asks any other personal information you can specifically request the FIT NOTE

I am vindictive and asked for everything just because they ignored the request by phone. IMO if more people did this when the DWP use bully boy tactics they would stop as it also means it is costing them to print out the copies.

:) Good news at last! Got my ESA payment backdated to September 2010, and paid into bank today. Sent in ESA50 just before xmas. Not been called for medical yet . Don't know if or when I will. No explanation of how calculation of £69.10 was reached. No award letter... But then again, I have been phoning DWP every couple of days to ask how my claim was going! I must have p****d them off, they gave in :laugh .
In one of these phonecalls, I said I knew I was entitled to 4 Cold Weather Payments (NR region). The DWP rep. checked and agreed that was the case. Said she would put in a request for it to be issued! Still waiting. Thanks to you all at Benefits & Work for being there for disabled people. You are doing such good work. I wish everyone else the luck I have had....so far......but then....I could get another ESA50 next week!

Just been told by DWP, that Cold Weather Payments only paid after 92nd day of incapacity if applying for ESA.So, CWP only payable from 20th Dec in my case.
Has anyone else been given the option to phone up for medical? I declined the offer, saying I would wait to be contacted.
