Is it possible that you got somebody to fill in the form for you, but you signed it after reading it?
Also is the content of the form consistent with your circumstances?

Thanks for the replies.
Honestly cant remember getting help with the IB50 & although some of the info is ok it doesn't mention almost setting the kitchen alight when trying to cook a meal or having to hold onto the handrail on the stairs & having to rest halfway up (flight of 12) nothing about breathlessness,pain & topping to rest. Poor memory, agression ect
things that i would have written. Should i put this down to a serious lap of memory.
Anyhoo on reading all the info again the nurse wrote that I put my foot up on a chair when putting my shoes back on, when infact Alex point out I wore slip on sandle type shoes as my back was playing up,my appearance was a bit unkempt as i could manage to brush my hair other factors wrote by the nurse were off as well. Phoned ATOS requesting a complaint pack and the stress of everything meant i couldn't concentrate at my CFS Course today.
Should i mention at my Appeal (anytime after 30/06/2011) will this invalidte the IB50 if i then start adding information to an already completed form?

I think the first thing to say it that you should get advice regarding your appeal as your case, in my opinion, is complicated by the discrepancies between what your IB50 said, and what you believe it should have said.
I assume that you must have had someone helping you previously, but failing that, your local CAB or Welfare Rights Officer should be able to help.
You should not add information to the already submited IB50 as I doubt it would be acceptable. You need your evidence to be clear and easily referenced, the more complicated you make it for the the Tribunal members to understand your case, the less likely it is to succeed.

You can't amend the IB50 but you are fully entitled to separately submit written evidence, including anything you feel should have been on the IB50. You can also point out that you have memory problems and do not remember filling in the IB50 so it should be understandable why you are giving information that was not mentioned on the form.
By all means point out all the errors and inaccuracies in the medical report, as well any in the DWP's submission.
Please read the IB appeal guide in the members' area.

I can only assume that I was having a really bad day with CFS and as said in earlier post someone else filled the form in for me,I assume if that is the case I must have read it failed to notice that it wasn't quite right signed then posted it. I will contact Welfare Rights Advisor 2morrow. Thats why Alex normally comes with me in cases like this so that if I forget something he reminds me. Has this ever happened before to anyone & if so how was it rectified

Thanks Derek am requestring my Hospital Records along with GPs & am sure CPN will write a letter for me.
Would it be helpfull to the Tribunal if I supply evedence my Hillwalking Leadership and photographic evedence of my hobbies that i can longer take part in?
