Seek advice to fill DLA form for serious alcoholic

Are there any examples or advice notes on how to fill in a DLA form for someone who has a serious drink problem?

I am trying to fill in a DLA form for a friend who has late-stage liver damage, frequent bouts of delirium when not drinking, etc etc.

I am a full member of the site so I can access the members' info area but I can't find much which helps with DLA claims due to alcoholism.
