I dont understand do you get the form first and then the phone call and what happens if you dont answer or reply by phone.my parent is my appointee will they phone them

I think the phone call is just to frighten advise you that you need to send the form back. I don't think you need to worry too much about the phone call or replying to it as long as the form is sent within the deadline.

I will look at the form again and make that point. I find it hard to try and explain how my mental health affects my physical health,but I will just be honest.
Your right,it's not rocket science,but I can now see how this damn form has freaked out so many people!

They do need to devise a better form.. So confusing,but that's what they want!
Aswell as having to fight my corner with this government,I am having to fight my local college,as I was thrown off my Access course last xmas,after a breakdown,even after asking them if I could do a majority of my assignments at home,until I was fit enough to go back to college! Would this be classed as discrimination? Sorry to jump off the subject a touch,but while I was here,I thought I would ask?

I wonder what would happen if someone with a physical illness was told they couldn't get points for depression too? Hmmm.
Regarding college, I really don't know - it sounds like discrimination. I suggest you do an Internet search on the Disability Discrimination Act. I'd also suggest you choose your battles, ie. don't try to take on too much at once. Does not help the anxiety one jot...
