Filling ESA form for someone else was created by judyjude
If I am filling in the ESA form for my daughter do I write it in the 1st person or 3rd person ie. I or She

Hi Judy,
If your daughter is going to sign the form and agree that its contents are accurate then it should be written in the first person.
However, if you are your daughter's appointee and she is unable to sign the form and agree its contents herself then it should be written in the third person.

I am going to fill it in in the first person. Do I fill the part 'For people filling in this questionnaire for someone else' or not. My daughter will be able to sign the form and agree with what is written.

hi,

Also how many times can I put 'See use this space' instead of ticking the boxes as many don't apply to chronic fatigue.

Please do not repeat questions as we are short of moderators and cannot always reply within a few hours.
The 'for people filling in this questionnaire for someone else' part of the form is optional but I think it would be preferable for you to fill it in, and as you said your daughter can check the form before signing.
There is no limit to how often you can say 'see use this space' but I would suggest that you tick the boxes where appropriate.
