DLA medical by ATOS

I'm sure this has been asked before but after half hour of looking I still can't find it so apologies if it is staring me in the face.

I have a medical at home by an Atos Dr a couple of weeks ago for my DLA.

I want a copy of the report to help me with appeals, would I write to the usual DLA Blackpool address or to the Atos place where the Doctor was based.

You should write to The Manager Disability & Carers Service, (DWP) Warbreck House, Warbreck Hill Road, Blackpool, FY2 0YE. Telephone 08457 123456. You are entitled to all the evidence used in deciding your claim, as well as the ATOS doctors report, who examined you.

thanks very much Michael, will wait and see what the report says, and if its any good I am going to use it / refer to it for my ESA appeal.

Have been waiting since March for my DLA decision.
