Capability Assessment Form & Moving was created by Tattie
Hi all
Looking for some advice.
My work cap ass form arrived last Friday to be completed by 14th Oct.
I move in two weeks time to a different county which means new doctors etc..
Can anyone perhaps give me a bit of advice best way to handle this - do I ask for extension to fill in the form due to circumstances?
Any advice welcomed
Thanks

Hi T,
I assume you mean your ESA50 Questionnaire form.
I would contact the DWP, and ATOS, in writing by recorded delivery post, and keep copies.
I would inform them of your pending move of house, ASAP and you could ask for an extension of time, but they don't have to grant this.
You may also be able to e-mail ATOS from their website on this link :
www.atoshealthcare.com/index.php?option=...ion&id=14&Itemid=288
You can telephone but would always back up in writing.
In the mean time, you could download the Pdf on line version of the ESA50 from this link :
www.direct.gov.uk/prod_consum_dg/groups/...lasset/dg_195544.pdf
You could then save it to your PC or USB memory stick, and fill it in and save and go back to it as you wish, then when required just print off and post.
This form will be the same where ever you move to, the ATOS postal address, will probably be different.
When you move, your claim may well be dealt with by a different DWP BDC.
I would definately try and sort it out now, rather than waiting until you move.
cheers

Thank you for your help and advice much appreciated.
I initially rang the number on the ATOS website marked DWP enquiries. They passed me over the the Resource Team where I spoke to a lady who has told me quite flatly there is NO extensions given. ever!!
So I guess I just have to suck eggs and get on with it.. even though I explained it was exceptional circumstances...

Oh and I have to add that she said that if it was late then, it would be sent back to where ever. I asked would this mean benefit being stopped and she said that it would be a matter for DWP not them.

Just to clarify.
There is no mechanism in the ESA process to extend the time for you to complete the ESA50, however, if you show good reason, the DWP can accept the late return of the form.
So I would make every effort to return the form by the due date, so you avoid any potential problems.
