Online ESA50

I put my nin on the top of each page and numbered the loose pages and added nin to those (additional info) when I finished my form it was early so I re read it several times and added extra notes where needed around the edges of the page! then I photo copied everything twice, one for me and one for g.p so he had a record of exactly what I had written should the dwp ask him for a Report. Remember to leave enough time for postal delays and send recorded delivery. Good luck with your claim, hope all goes well. I thought I wouldnt get through after failing first time, but when I reapplied I got hr mobility and no medical, that was after filling in the forms step by step using the members guides.

Rymans sell those plastic clips for 35p each. They hold around 30 plus pages together. They fit down the length of the A4 page.

I swear they were made for online DWP forms
