DLA renewal forms was created by hassled mum
After applying for dla in January this year, it was awarded at hr mobility in March. with my claim I submitted a lot of medical evidence and paperwork, since my claim went in I have since been prescribed a wheelchair (non active user as I cannot self propel for any distance apart from a change of postition)have been prescribed ankle braces by the orthopods, I also have a formal diagnosis of my condition.
my question is, I know that I will need to include all of the new evidence with my claim and show how my condition has deteriorated over the last eight months, but do I need to resubmit the reams of evidence that went in to the dwp in January?
I have the guides printed off from the last application, but have there been any changes since Jan 2011 in the application for dla?

Hi hassled mum

If I read your post correctly, you say you were awarded HRM DLA in March, now you have been prescribed a wheelchair which you cannot self propel and have ankle braces.

You wish to submit new evidence to show how your condition has detiorated. Is this because your HRM has ceased, (only awarded for short period maybe)? If you are in a position where you are now unable to go out you will no longer be entitled to HRM and have a duty to inform the DWP of this.

Or, am I reading your post wrongly? Please clarify.

Hi Michael, I am currently in receipt of hrm dla awarded for one year due to expire on Jan 1st 2012 I received forms yesterday (not date stamped!) asking if I would like to go ahead and put in another claim/renewal for dla and if successful to run from when my current claim expires in January 2012.I am able to walk very short distances using two sticks or crutches depending on the need that day (distance from door to car and maybe a little further but in high levels of pain) I am also able to drive an automatic power steering car for short journeys, therefore still able to go out to the g.p or station for hospital visits, husband does any journey over 20 minutes. The wheel chair was prescribed to enable me further access out doors, to be able to go out for longer trips but because of joint and heart issues doc does not want me to self propel (too much pressure on heart and shoulder/wrist joints) so I am not house bound and still rely on my husband to accompany me on any outings. The question was, if I submitted all medical outside agency evidence in Jan this year do I need to resubmit it with the new claim? I have new evidence to add to the new claim anyway as mentioned above. The brace is to try and stabilize my ankle whilst walking as that is one of the causes of my falls along with other issues that also cause them.It will also slow down the ongoing damage to that joint. so the crux is yes I can still go out with support, do not need chair indoors as I sleep down stairs and use sticks inside.it's just whether or not to include a repeat of the info already submitted.

Hi hassled mum

A DLA renewal claim, is basically treated like a new claim, therefore you should include all relevant supportive evidence. The B&W guide in the orange section "Disabilty Living Allowance Renewal Claims" was published October 2006.

onwards and forwards to rev up the photocopier then. I will let you know how I get on, last time it was awarded without medical, but who knows what will be!

I'm afraid its the tedious and repetitive form to complete. Copy everything and send by recorded delivery. Put your name NI & number all pages. FYI I was always awarded without EMP before, this time they sent their doctor, who went through a mobility test even though I had an indefinite award of HRM!

The renewal form is meant to be a shorter version of the full form, difficult to see any difference imo, unless you have the short 4 page version which is being trialed in certain areas of the country, when the advice from B&W is to download and complete the full form anyway!

Make sure you include all up to date information and that previous problems you needed help with still exist. remember its the help you need and not just the medical condition.
