I lost my form

I was ready to post my ESA50 off and to my horror I couldn't find it. It wasn't where I had left it
I needed a new one and I phoned the ESA people to send me another form asap.
Will I get penalised for this?
I am so very very worried, has this ever happened to anyone before. I cant believe this has happened to me.
Any advise would be so very helpful

I have made photocopies of the form so I know what to write again, But I feel so foolish and wondered if my benefits will be affected because I phoned them and asked them for a fresh form.
It is not due to be back to them for another four weeks, but I suffer from bad anxiety and I feel so foolish and worried now incase it affects my benefits.
I'm so worried

Hi Stu,
You can download a Pdf version of the ESA50 from a link that I provided in this thread from 3 weeks ago. :

You should get 6 weeks from receipt of your ESA50 through the post to fill it in , and return it.
Are you that close to your deadline return date?.

Stu,
Don't worry too much.
As long as either the normal version of the ESA50, or the online Pdf version is completed and returned to them by the deadline date that they have given you, they can't penalise you.
If you can, the online version is a lot easier to complete, as you can save and alter it as you wish, and then simply print it off and post it before deadline.
You will then have a ready made exact copy left on your P.C. which you could also backup to USB memory stick, or other external hard drive, as well as keeping a hard copy if you wish.

I have very bad anxiety and I panicked and just phoned them up, completely forgetting you had already supplied a link for me (I was so anxious)sorry ..
I am sure I can return it within the original deadline
But I worry about everything
I have just over 4 weeks to return it -
My biggest fear is will this affect my benefits by asking for a new form?
