Yes, it is absolutely fine to use the computer to type your answers to the questions. As you suggest, write something like 'see additional sheet x' in the text box.

I don't think it really matters whether you keep each question separate. Do whatever you think makes things clearest.

We don't advise trying to stick the answers to the form as that limits the space you have and also potentially makes it harder to scan into the system.

I always print on one side of the sheet - simply because it is easier that way!

Hello again.

For the PIP2 form questions - do you need help to do a certain thing?

If you are going to score points for this answer presumably you should always tick 'yes'. I know that includes help that you need but might not have.

I notice in the guide there are sample answers for somebody needing help with bathing. However, because they have OCD, they say another person trying to help them bathe would make it worse.

Will they still score points when they put this answer?

I notice there was another answer on mixing with people where someone said having another person trying to help with this could sometimes make things more stressful for them.

Again, will this score you points?

I ask because although I can't do a number of the pip tasks reliably for some of them another person trying to help me could possibly make it even more difficult for me.

Think of it in these terms, all of them would score points if relevant.

- First of all would an aid or appliance help you to complete the activity? If does then why is this and if not, why not?

- Do you need to be prompted to do an activity? if you do why is this? If you don't receive prompting how do you manage with the activity, is it simply not done or if you do manage it how do you do this? Is it done but to an unacceptable standard or does it takes you an unreasonable time?

- If you had assistance from someone would it help? If it does how? Again, how do you manage if you don't have the assistance?

- Do you need supervision? Why? How do you manage if you don't get it, do you not attempt activities or if you do how do you mitigate the risk of harm to you?

If you don't think that the tick boxes allow you to answer the question in the way that you want then mark them "see below" and answer in the comment box or attached pages.

An example for me would be due to my mental health I can't manage to cook a simple meal so I heat a ready meal. I would find someone trying to prompt me or assist me in cooking a meal added stress and infuriating.

I was planning to tick yes I do need help but then say this.

I suppose I could put see below instead of saying yes or maybe it doesn't matter if I ticked yes.

But from what you have posted you do not need help as it does not allow you to complete the activity!

Also you need to explain why you your mental health problems prevent you from preparing and cooking a meal.

Yes, I would explain in more detail why my mental health means I can't cook.

But if I say help would make it worse does that mean I would score no points then?
