Hi Can you advise me what is the correct thing to ask for from dwp.for the assessment points/score.

I want to keep for our records
put in the support group after assessment for ESA, also what should I ask for the pip after assessment.

I was put in support group no assessment, so I don't know what papers to ask for there either as no assessment
Can anyone help me with the wording please.

For bot it should be possible to just ask for the assessment form by contacting the DWP office dealing with each benefit.

For PIP the report is called a PA4 if you had a face to face and a PA3 if you did not. For ESA it is an ESA85 and an ESA85A.
