Hi Everyone
Well DLA - PIP transfer letter received!! can i ask, i have noticed on the forum the mention of doctors note's, could you please explain exactly what this is? is it a form you give to your doctor to complete to send with the PIP application? (if the doctor would be prepared to do it) I would appreciate any help.
Also i independantly applied for a Blue Badge through our local authority and passed assesment due to limited mobility, should i mention this in PIP application or would it not carry any weight?
I have been on Low Mobility and Middle Rate care DLA indefinately for several years.
I have mental and physical conditions and i hope for a paper based assesment because of my illness's, and hope i have enough medical evidence to support this, i know they are rare so will have to see.

There is a GP Notes document in the PIP area to complete. they can either complete the pages that they think are relevant or use it as a template to write their own letter to the DWP.

Yes include information about your Blue Badge, the criteria is slightly different from PIP but it will certainly help with showing that you meet the criteria for the Moving Around activity.
If you are hoping for a Paper Based Review then input from your GP will be critical because you are very unlikely to get one without it.

Thanks Gordon, another quick query, i have been in the SG of ESA for several years, the last two reviews were done without a F2F, as i sent alot of supporting medical evidence and i believe i am in the SG because of Reg 35? I think that is what it is called, should i mention this on my PIP form? any advice would be greatly appreciated.

There's no reason not to but don't expect to much of it PIP has no equivalent Regulation you may be able to show that the Work Related Activity that is covered has some relevance to the PIP criteria but it will be generalised at best.

Hi Everyone
Submitted PIP form and lots of supporting medical evidence, i suffer with clinical depression, anxiety, panic attacks, severe Irritable bowel syndrome, fibromyalgia and arthritis, comprehensive specialist/doctors/ letters and prescription list and long covering letter, consultant bowel specialist even put on his letter, due to the sensitive subject matter of my illnesses a paper based assesment should be done, also my doctor had mentioned it in her letter, which was also very detailed on how my illnesses affect my life etc, i had not heard anything so my friend rang the DWP (i don't handle calls very well) and they said it had been passed to IAS, so she rang them, to cut a very long story short and several hour's on the phone, they said that a HCP had looked at the claim form, substantial evidence and they needed to do a home visit, my friend escalated it to a manager explaining that it would cause me significant psychological distress, she also went back to the DWP also and they sent IAS a note re the evidence and request, (the DWP said the could not understand why a F2F was necessary based on the evidence and it is very recent evidence) IAS came back and said another HCP had looked at everything and because of the complexity of my illnesses they had to do a F2F despite my friend explaining the affects it would have on me mentally, which in turn would affect my bowel and how insular i was, they were adamant, nothing they could do, F2F has to go ahead!!, the IAS manager said he would sent a complaint's pack and it would help with their feedback!!, he said i could refuse the assessment and the file would be returned to the DWP but i would probably have my case closed, so this friday it is? terrified, anxious and really don't understand despite my medical professional's detailing about a paper based assesment for my wellbeing, their HCP say's no. Don't hold out much hope of getting PIP, husband has prostate cancer so has a hospital appointment and can't be here, friend also has an appointment so will be on my own, considering cancelling as i have been so unwell with all of the stress and anxiety over it, any advice would be greatly received.

I suspect that your letters have not been sufficiently forceful about your needing a Paper Based Review.
Would your GP write a letter categorically cannot attend an assessment, if they would then you could also try involving your MP.
