IB to ESA checklist query

Thanks bro - I'll digest what you are saying and familiarise myself with all that you've mentioned. If my GP is reluctant (I think he'll be OK), my surgery is a really great place with very helpful staff if I need to go down the route of getting access to my records.

I'm sorry if I missed this really basic info but if you fill in the online version of the ESA50 and print it up, is it OK to send your print-out, or do you also have to fill in and send the paper version of the form which ATOS sent? Apologies for being so dense. My head is reeling from filling in the form

Hi T,

You can simply print off and send the Pdf version.

You can keep the nice pink paper one that you received through the post as a souvenir. (Although I could think of more appropriate uses for it)

I just wanted to say Thank You for the help you've given me. I have fiinally finished filling in my form and printed up my supplementary evidence. I've got all my evidence copied and numbered and I'm ready to do a copy for the decision maker. I don't know what everyone else does but I mush have been through the guides 10 times before I was satisfied with the answers. I'll let you know what happens. Cheers!
