I will have to raise the problem with the site owner as it appears that the numbers on the standard claim form are different from the DLA80 that you have, I had hoped that they corresponded to the old guides, but these are still one off!
We will not be able to provide a solution quickly so I am afraid that all I can suggest at the moment is that you match the form questions to the guide ones by the text rather than the number, these should be the same.
