Hi Applecharlie,
Good advice from LL26, I found that my GP & physio were both very happy to write strong supporting letters and likewise with my neurophysio and consultant. The more supporting letters you can get, the better.

Although I have seen it said you do not need to send them stuff they have had before, personally I don't trust them to bother looking back, whatever they said on the phone. So I put it whatever is the most recent that covers the effects for each descriptor ie is still relevant, and yes, I do reference each proof doc point by point. Some of them are none too bright unfortunately and it's been said they are on some kind of piece rate. So I am creating a proof trail that will stand up to a Tribunal later if it should be necessary - but hopefully giving "chapter, verse, subsection and paragraph," though as concisely as you reasonably can, will avoid it having to go that far. Thanks.

I had a good telephone appointment with my GP today which covered (unintentionally) some of the things PIP covers. So hopefully, if she documents the call correctly, it will help.

I need them to pull their finger out and get my records to me. I put the request in at the start of January and only today they told me there are delays and I should ask the DWP for an extension. Not helpful at all.

I have completed as much of the form as I can but I still lack my records from my GP. I put in the request when the PIP form was on route and chased it twice. This morning, I've had an email back from my last chase up basically telling me in polite terms to get off their back and they will get the records to me when they get to me.

Apart from the records being evidence that my condition has not changed, there are key dates as to when I was referred to certain departments etc (which in hindsight I should have made notes on) which I don't have without my records and my GP surgery is being unhelpful in not providing them to tide me over until my records are sorted.

So this leads me to my problem, do I send off my form with the really old evidence with a cover letter explaining my difficulties getting my records (with the very official looking email as evidence) or do I contact the DWP and say there is a delay and my form will be back shortly?

You can't just tell the DWP that you form is going to be late, you have to ring up and ask for an extension. Explain the reason why you need one (if they ask). They should be understanding because they know GP's currently have other priorities. If they refuse the extension, then you will have to return the form with a note saying that you have been unable to get access to your records, and you will send on further information as soon as you receive it.

Thanks BIS for the reply. I have already been working on the basis of doing this as I know the sooner I get my paperwork back, the sooner they can get my claim sorted and less risk of my money stopping in April.

I just fear that without my records, they are just going to go, no proof and stop my award and then it's an uphill battle from there.
