I used the new guide, then info on review at the very end of the guide.
It can only be general as regards the form, so little space and there is no date on it just a series of what look like barcode numbers, over locking in bottom left. The form says PIP.1043 in the bottom right. Sequence is altered so I typed my info into a word doc I called Sheet A, with each question number and heading. In the boxes supplied I put dates of change, and what they ask for which is really pinicky, one over all heading would have been enough.
The answer to the questions was 7 pages long, for me, but I have 4 substantive new diagnosis since 2015, my last PIP application. With supplemental material they got a thesis delivered today.
I would have liked to see the face of the person receiving it, saw the signature, signed by persons name, but the signature said Covid, in looped writing.
Interesting does covid sign all deliveries or just giant chunky ones. But I do know who covid is, they gave their first name to royal mail so that's all right then.
Please let me know what happens to yours and good luck.

I have not yet seen an on line form myself yet although we have had a few members on the forum who have.
Thank you for your reply, information from members is very helpful, not only for us mods but also for fellow members.

it was paper, very thin. We are in the process of doing our sons, bit of a nightmare given his GP will not release anything without an appointment due to mental health, they say. First appointment 6th Dec because its not medical.
Our son was one of those who they did not backdate regarding mobility. He did get standard rate in 2018 and the letter from them saying they had looked at his claim again in 2019 regarding his prior 2014 claim did not take into account any information regarding the evidence submitted on agoraphobia, social anxiety and various other evidence at the time from mental health teams to say he was unable to leave the house without extensive support and medication.
Unfortunately he was far too ill to challenge the decision and so was I. Just looking through it all today and its really annoying what they put in the letter, sort of makes it seem like a review and he should be very glad he now has enhanced care with standard mobility.
They know no shame.

Hi again and thank you for so much for staying with this for me .. and thanks to the others for their input to ... I'm just going to add all the new stuff thats going on with me since April and I'm adding all the old stuff too ... Whoever gets the happy task of serving me deserves a lengthy war and peace novel to brighten their day at the office ... Thanks again everyone
