Hi,
I need some advice...
I have recently received pip1043 review form to complete.
I have already requested more time to complete form and return it. This has been granted.
The reason for my request for extension time is because time was lost over the Christmas bank holiday period.
Since my original claim 3 years ago my health has deteriorated & I have now been diagnosed with fibromyalgia along with my other existing health conditions.
When I originally filled the claim form 3 years ago in I failed to include that I suffer with severe anxiety & panic attacks, this is something that I have suffered with for quite a few years & it is included on my GP records. Should I now include this on my review form.
I am able to to travel in my local area approximately a 10 mile radius of my home, but outside of this I need to be accompanied by another person when traveling to unfamiliar places & areas.

Hi Elizabeth

To ensure you receive the benefits that are due to you, you have to include all conditions and illnesses, as well as how they affect you.

This means that you should include on the form all of the conditions and illnesses that you reported 3 years ago where you still continue to suffer from these. If they have worsened then make sure you say that.

If you have a formal diagnosis of fibromyalgia then include a copy of that in your response together with how that affects you. Remember that you should write about your flare-ups not just your day to day condition.

Similarly you should definitely include the GP records data about your anxiety and panic attacks and again add how they affect you..

On the subject of travel, if any of the people who support you on longer journeys are willing and able to provide you with written confirmation of their input and how they help you including what they see that you find difficult then you should include this in your return.

As a general point, if you are using any aids to enable you to perform your daily tasks please do remember to list them all. This can include fixtures such as grab rails, walking aids, even down to aids to help squeeze the toothpaste out of the tube. You can also consider things like fat grip cutlery and cooking utensils where you use these and things like perching stools for preparing food. Basically for each section of the form don’t forget to mention all aids you use.

I hope this helps and I know it’s a lot of work but it is necessary so that you get the correct benefits.

Are you saying that I should complete this review 1043 form as if I were making a new pip claim for the first time..
The reason I ask this is, Pip are already aware of my existing, still relevant health conditions and having to use aids daily ..these were included in my first pip application on changing over from DLA. I was awarded the appropriate points, scoring 8
Pip already have a stack of evidence ie, copies of consultants letters, medication prescriptions, hospital day case procedures and treatments, hospital admissions and operations.
The only thing that has changed is being diagnosed with fibromyalgia, I have written documentation confirming diagnosis from my pain consultant.

My personal experience is that, even for a review, it does no harm to include, update where necessary and confirm the status of all previous conditions, medication, aids, etc..

Doing this clearly answers all of the reviewer’s questions so they are fully informed about your current status without needing to reference previous paperwork.

It will also save unnecessary questions from them as to, for example, has your medication changed since your previous assessment.
