I have just received form pip 1043 for a pip review , haven’t completed a form since 2015 . This form is so much smaller eg ‘since 2016 what has changed ‘ so
1) is this a more basic tick box form or have they just made it look easier and we need to be as thorough as the last ?
2) I can’t tell if this is the new PIP form or the old , help !
3) Its taken 2 weeks to reach me so I only have two weeks left ?

A pip 1043 form is another name for an AR1 short review form, it should still say AR1 at the bottom left of each page. Its best to treat all reviews as you would a new claim.
If nothing has changed since your previous pip assessment, its best to explain in detail what has changed and what has stayed the same and how your finding each task.
If you have an up to date review of your treatment, therapy and medication it could be help full to include this also.
it is ok for you to write/type any extra information and attach them to the AR1 form, make sure each extra page you send has your full name and NIN at the top of each page and each is numbered that is sent with your review form, make sure you clearly mark on the form where your answers are and on the relevant page that you are answering questions.
If you feel you do not have enough time to complete this form you should be able to arrange a extension by calling the dwp and explain that you need a bit extra time.
I hope all goes smoothly for you, try and fill in the form over a few days and take plenty of breaks as from previous experience i know this can be very stressful attempting filling the form all in one go.
