I have been on dla/pip for the past 25 years and have always got medical evidence from my GP . My GP has gone on maternity leave so tonight went to see the person covering .
I went in and started by saying I needed some help with paperwork and was told ‘ I’m running a surgery I can’t do anything with paperwork’ … I won’t go into anymore but this set the tone … basically he argued all the way through saying that they don’t provide evidence they wait until it’s requested .
I also asked for him to review my medicine list because the review was a year old and he told me that it didn’t make any difference whether it was out of date or not …
I’m gutted
How can I send in a pip review without any supporting evidence ?

I can understand your frustration! However it seems unlikely your new GP will be particularly helpful anyway!
Firstly is there another GP in the practice you know and is perhaps more friendly! Maybe a nurse or physio elsewhere who could help?
Failing that, you are entitled to a copy of your medical records. You should write in to your GP asking for your records perhaps back until your last PIP review. This will at least show your medical problems and medications, and would evidence any change in meds or increased doses. There shouldn't be any charge for a copy of your records. If you obtain this and it isn't helpful there is no obligation to send a copy to the tribunal.
In addition or instead, you could ask a relative or friend to make statement about how your disabilities affect your ability to do PIP activities.
Aside from PIP claims, you could also consider making a complaint about your new GP. You may also wish to try and see another GP when you next need an appointment, if you feel you have no confidence in the new GP.

Hi fmoss1, in addition to all LL26's sound advice may I recommend 'create-your-own' evidence. This is to keep a diary, there's advice on this in the Guides, and in my experience of our daughter's claims, it can carry a surprising amount of weight with assessors. You can use the template or make your own, it can even be written by someone else; I made the last one by trying daily to phone daughter [we no longer live near] and logging frequent lack of answer as evidence of her chronic fatigue.

Hi. Depending on the size of your practice, there may be a Practice Manager you could contact with your concerns. Also, I would chat to your Chemist, and explain you need a med review. Depending on your meds, these should be done periodically anyway. What a complete ****.
