PIP Review - more than one health professional was created by CrazyCatLady
Hi Peeps - just starting the long process of going through my Husbands PIP Review form - under section 2 - it asks to name the main health professional for your condition. However, on the original PIP Claim form back in January 2018 - it asked for professionals - giving the ability to name 3 professionals which covered 80% of his conditions. Also under Section 2 on the review form they ask for consent to contact other health professionals - which is fine - but am I correct in the assumption that they will look back at his original claim form from January 2018 for this information?
In order to overcome this one health professional should I do a continuation sheet for this section 2.

Hi CrazyCatLady, ( I like your handle)
If you have more than one main health professional then add details under othe information, I would also reference this other information under section 2.
Just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
I would not split your answers across the form and attached pages, do one or the other for each activity (not question), splitting it risks the second part of your answer not being read.
Attach the pages to the back of the form using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

I’ve sorted it and added a separate list of all the consultants as supporting evidence - should have thought of that first before posting
