Good afternoon
I'm still having problems filling out this review, my anxiety is through the roof so it's making it more difficult,when just think I've got my head around it I think about something else and keep second guessing have wrote my answers wrong.
so my questions are-
in the boxes "tell us if something has changed and approximately when" I've wrote "information continued on extra paper" I'm not sure if this is the right thing to write in these boxes could someone advise me or an example what I could add to it just so the extra information is taken into account,
but with the extra paper I've took the approach of completing like the original form,this is my first review form and finding it more difficult to complete.
I have mental health problems and mobility problems too,I've had no mental health input for years CBT talking therapy and numerous amounts of counselling but have no written proof of this do you think this is worth mentioning
ive got till the 18th of February to send it back as already asked for an extension but that date is fast approaching,
thank you in advance any help I'd be greatful.

Make sure you number all of your extra sheets. Put the relevant sheet numbers into the relevant boxes. I recommend you list them like “additional Sheets 1,2,3 apply to this question” rather than “additional sheets 1-3 apply to this question.
You say you’ve had no mental health input for years, but then go on to mention CBT, talking therapies and counselling. These interventions are a form of mental health input and if they have happened within the last 3 years then you should certainly mention them together with details of the providers. The DWP can then contact those providers to request evidence.

I totally agree with what you have said. The stress this form puts you through and the worry that you answer every question in a way that can’t be picked apart and used against you…..my partner had to ask for an extension yesterday after we had a letter saying they hadn’t received my review form an it’s due on the 22nd February. They seem to think they give you a month to fill it out. By the time it gets to you, mine took 8 days, and then having the where is it letter adding to the stress, it’s just over two weeks. The lady on the phone said it needed to have been posted 9 days prior to the date it was due by. So the extension they gave me was until 5th March. They also said they don’t sent out letters to confirm this extension so it’s vital you get the persons name and note the da and time if the conversation. So at least I get another few days to try and get it right. I like you have basically used it as a new claim form. That way in my view they have all the facts in front of them. I’m also getting help in typing it up and print the answers on paper as our handwriting isn’t the best, again in my view it must help. Over two weeks worrying and sleepless nights……so I fully understand how you feel.
