Do you know if I can get a report on the first phone assessment they done 6 weeks ago

Hi Eric,
certainly you can try and ask for the form- it's called PA4. However, if they are asking you to do another assessment, it may be that the original form etc has actually been lost, so there won't be a copy. Or alternatively the assessment was so badly done that it needed to be redone. (I can't think why else you would need a second assessment.) I would be tempted to phone DWP ask why a second assessment is needed and ask for the PA4 for the 1st assessment. You could explain that you found the first assessment very difficult (if this is true) and felt that you covered everything, but this stressed you out and caused a lot of anxiety etc - you would rather not have to go through the experience again. (This approach might encourage a more sympathetic DWP decision maker not to require a second assessment - but of course that might then mean relying on the 1st (inadequate) report!
If DWP say they have lost the form, or admit the first assessment was poorly done etc, then you should write a complaint to DWP and copy to your MP. Make and keep notes of all phone calls and letters. If you wish to make a complaint - mark it clearly 'Complaint' . Make sure your full name & NI Number are clearly marked on all pages of any letter sent in.

Hi thanks for your reply I did phone them and asked could I get a copy off the phone assessment I done 6 weeks ago and asked why it had to be done again she said that I couldn’t because it wasn’t completely done they needed more evidence on my medication so I asked her should this not off been done by the first assessor she then didn’t know I said it had to be done again by someone else .

Yes you can get a copy of your assessment usually after 48 hrs, contact DWP and ask for a copy of the assessors report called a PA4.
