I remember this happening with my late mother. She just contacted the number on the form and I think at the time (although it could have changed since then); there was a short form that you have to fill out along with verification from your Dr as to why it would be detrimental to both yourself and to the purpose of serving on the jury (i.e. unreliability due to pain of getting up and to the court would have an impact on the case).

You may also like to know these requests for jury service are done totally at random as my mother actually received another one a few years later (she just repeated the same with the Dr putting no change.

I've just used this link to ask for someone to be excused. They don't qualify to be exempted (ie never permitted to do jury service). But you can ask to be excused on health grounds www.gov.uk/jury-service/delaying-or-bein...ed-from-jury-service
If they agree, your name is taken off of the list from which jurors can be selected for 1 year.
After a year, if you do get summonsed again, you can just ask to be excused again.
